New Patient Registration

Banner-ImageThe practice boundary covers all parts of King's Lynn and also includes Ashwicken, Bawsey, Black Borough End, Castle Rising, Clenchwarton, Eau Brink, Fair Green, Gayton, Grimston, Leziate, Middleton, North Runcton, North Wootton, Pott Row, Roydon, Saddlebow, South Wootton, Tilney All Saints, West Lynn and West Winch.

You will be asked to call in to collect a registration form, information pack and to make an appointment to attend for a blood pressure check if aged over 45.

We are currently taking new patients on to our list.

We are a PMS practice which means you do not need to register with a 'named' GP. You are free to see any GP that you wish.

Patients requiring a GP who are unable to gain voluntary registration with a local GP should contact;

Online Access

While registering with the practice register with online access. It allows you to use an online booking service to do the following:

  • Book and cancel appointments online
  • Request repeat medication
  • Update demographic details
  • Complete questionnaires
  • Use SystmOnline apps

Simply pop into the surgery and speak to one of our helpful receptionists about online access and they will be able to supply you with all the paperwork you need.

Registration With The Practice


When you register you will be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.

Medical Questionnaire For New Patients

Fill it out and bring it in with you to the practice with some photographic identification.
